| As a St. Luke's Family Practice
benefactor, you have direct access to your physician in person
or by phone, fax or email. Smaller doctor-patient ratios allow
for same day service. St. Luke's Family Practice uses technology
to keep costs down, track your personal health history and allow
state-of-the-art information sharing. Benefactors must maintain
high deductible or "catastrophic" insurance coverage
for non-primary care services such as hospitalization, specialty
consultation, pharmacy, lab and x-ray services. Click

Each physician will only be able to accept
about 300 patients. To guarantee that you and your family's
Benefactor application will be considered, we request a completed"Letter
of Interest" and a fully refundable deposit of $100 per
person mailed to St. Luke's Family Practice, 1400 Florida
Avenue, Suite 109, Modesto, CA 95350.
St. Luke's Family Practice is a federal tax-exempt
